The form in question has to have these flags (Forms Management),,,Active, Not Archived,

Credit Application, and Quick Credit App. It also has to have an filename+_BLANK.pdf file in the location generally misc.

Ex. CREDAPP_123.pdf you woul also need a CREDAPP_123_BLANK.pdf as well.
